Informed Consent, Clinic Policies & Patient Agreement
By signing below, you acknowledge that you have read, understood, had the opportunity to ask questions about, and agree to be bound by iCollab Healthcare's Informed Consent, Terms of Use & Patient Agreement, and Privacy Policy. This agreement applies to all services you receive from the Clinic, whether in person, by telephone or virtual appointment, or through our aesthetic services.
1. Services offered
iCollab Healthcare (the "Clinic") offers medical and health care services, including diagnostic assessments, medical treatments, health management, virtual/telephone care, and aesthetic (cosmetic and therapeutic) services across our Surrey and South Surrey locations. Some services (including certain aesthetic and other non-MSP services) are not covered by the BC Medical Services Plan and are your financial responsibility.
2. Risks & benefits — assumption of risk
You acknowledge that any medical or aesthetic treatment carries inherent risks, and that no guarantee can be made as to the outcome of your treatment. You understand that benefits may not be immediately apparent, that results may vary between individuals, and that alternative treatments may be available which you may choose to pursue instead. You voluntarily assume the risks inherent in the consultation, services, and treatment you receive.
3. Consent to treatment & electronic communications
By proceeding, you provide informed consent to the treatment and services discussed with you. You also consent to communication through electronic means for personal health services and information, understanding the inherent risks of such communications (including telephone appointments, emails, text messages, web forms, and messaging applications such as WhatsApp), consistent with CMPA guidance. Electronic communications can be intercepted, delayed, misdirected, or accessed by unauthorized persons. Your physician and the clinic may use artificial intelligence (AI) tools to assist with your care — such as clinical scribing, documentation, and administrative tasks — which support, and do not replace, your physician's clinical judgement.
4. Confidentiality & privacy
Your personal information and personal health information are collected, used, and disclosed only as necessary to provide and coordinate care and to operate the clinic, and are handled in accordance with British Columbia's Personal Information Protection Act (PIPA) and applicable Canadian privacy legislation. You may request access to, or correction of, your information by contacting the clinic.
5. Payment
Services may be covered by the Medical Services Plan of British Columbia (MSP). Any charges not covered by MSP are your responsibility, including co-payments, deductibles, and charges for services not covered by MSP.
6. Release and save-harmless (indemnification)
To the fullest extent permitted by applicable law, and in consideration of receiving consultation, services, and/or treatment from the Clinic, you agree to release, indemnify, and hold and save harmless the Clinic, its physicians, partners, officers, directors, employees, agents, contractors, and representatives (collectively, the "Released Parties") from and against any and all liability, claims, demands, actions, or causes of action, whether at law or in equity, arising from or in connection with the Clinic and/or your consultation, services, and/or treatment, to the maximum extent permitted by law. This commitment extends from the corporate level through to individual physicians, employees, and contractors. Nothing here excludes or limits any liability that cannot lawfully be excluded, including any non-waivable professional, statutory, or regulatory duty owed to patients under British Columbia or Canadian law.
7. Revocation of consent
You have the right to revoke your consent to treatment at any time. Such revocation will not affect the legality or validity of any actions taken prior to the revocation.
8. Zero-tolerance for abusive conduct
The Clinic maintains a zero-tolerance policy. Any disrespectful or abusive language or behaviour directed toward staff or physicians will not be tolerated, and may result in discharge from the Clinic and refusal of further service.
9. Dispute resolution & governing law
In the event of a dispute, you agree to first attempt to resolve it through good-faith negotiation before pursuing any other remedy. These terms are governed by the laws of the Province of British Columbia and the laws of Canada applicable therein.
