These Terms of Use and Patient Agreement (the “Terms”) apply to your use of this website and to your use of any iCollab Healthcare (the “Clinic”) service, whether you attend in person, are seen by telephone or virtual appointment, book online or by phone, or use our aesthetics services. By using our services or submitting any form on this website, or by booking or attending an appointment, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy.
1. About this website & no medical advice
This website provides general information about the Clinic's locations and services. It does not provide medical advice and is not a substitute for professional care. Content on this site is for general information only; always seek the advice of a qualified health provider with any questions about a medical condition. In an emergency, call 911 or go to your nearest emergency department.
2. Services offered
The Clinic offers medical and health care services, including but not limited to diagnostic assessments, medical treatments, health management services, virtual/telephone care, and aesthetic (cosmetic and therapeutic) services. The specific services you receive will be discussed with you and documented in your medical record.
3. Risks and benefits — assumption of risk
You acknowledge that any medical or aesthetic treatment carries inherent risks, and that no guarantee can be made as to the outcome of your treatment. You understand that the benefits of treatment may not be immediately apparent, that results may vary between individuals, and that alternative treatments may be available which you may choose to pursue instead. You voluntarily assume the risks inherent in the consultation, services, and treatment you receive.
4. Consent to treatment & electronic communications
By proceeding, you provide your informed consent to the treatment and services discussed with you, and you consent to communications through electronic means for personal health services and information, understanding the inherent risks of such communications (including telephone appointments, emails, text messages, web forms, and messaging applications) consistent with CMPA guidance. Details are set out in our Privacy Policy.
5. Payment
Services may be covered by the Medical Services Plan of British Columbia (MSP). Any charges not covered by MSP are your responsibility. You agree to pay all charges for services rendered, including co-payments, deductibles, and charges for services not covered by MSP.
6. Release and save-harmless (indemnification)
To the fullest extent permitted by applicable law, and in consideration of receiving consultation, services, and/or treatment from the Clinic, you agree to release, indemnify, and hold and save harmless the Clinic, its physicians, partners, officers, directors, employees, agents, contractors, and representatives (collectively, the “Released Parties”) from and against any and all liability, claims, demands, actions, or causes of action, whether at law or in equity, arising from or in connection with the Clinic and/or your consultation, services, and/or treatment, to the maximum extent permitted by law.
This release and save-harmless commitment is intended to extend from the corporate level through to individual physicians, employees, and contractors of the Clinic. Nothing in these Terms operates to exclude or limit any liability that cannot lawfully be excluded or limited, including any non-waivable professional, statutory, or regulatory duty owed to patients under British Columbia or Canadian law.
7. Revocation of consent
You have the right to revoke your consent to treatment at any time. Such revocation will not affect the legality or validity of any actions taken prior to the revocation.
8. Zero-tolerance for abusive conduct
The Clinic maintains a zero-tolerance policy. Any disrespectful or abusive language or behaviour directed toward staff or physicians will not be tolerated, and may result in your discharge from the Clinic and refusal of further service.
9. Dispute resolution
In the event of a dispute between you and the Clinic, you agree to first attempt to resolve the dispute through good-faith negotiation before pursuing any other remedy.
10. Governing law
These Terms are governed by and construed in accordance with the laws of the Province of British Columbia and the laws of Canada applicable therein.
11. Appointments & third-party booking
Online booking is provided through a third-party platform; your use of that platform is subject to its own terms and privacy practices. Phone and virtual appointments may be conducted by telephone, including from a blocked or unknown number.
12. Intellectual property
The iCollab Healthcare and iCollab Aesthetics names, logos, and site content are the property of iCollab Healthcare and may not be used without permission.
13. Acknowledgment & changes
You acknowledge that you have read these Terms, that you understand their contents, that you have had the opportunity to ask questions, and that you agree to be bound by them. We may update these Terms from time to time; continued use of our website or services indicates acceptance of the current Terms.
